Raise Awareness About Sepsis in Your Facility this Month!

Each week, Telligen will provide interventions, tips and resources to protect your residents.

 

Response to sepsis begins with screening, regular assessment and early recognition.

 

A process map (or workflow) serves as a practical, quick guide for managing sepsis in your nursing home. If sepsis criteria are met, engage a rapid response. You can create a process map based on your protocol and capabilities. Refer to INTERACT®’s Guidance on Management of Possible Sepsis for further information. You can also create a process map individualized to your facility using Telligen’s process map template.

Telligen invites you to participate in the Sepsis Awareness Month Storyboard Poster Challenge! Highlight the initiatives your facility is taking to promote awareness during this critical month. Please incorporate your process map onto your Sepsis Awareness Month storyboard poster.

 

Submit a photo of your poster to Telligen before Wednesday, September 25 to nursinghome@telligen.com. All storyboard participants and winners will be recognized on the Telligen website.

1. Consider developing or updating a sepsis workflow based on your facility protocol.

  • If you don’t already have sepsis protocol for your facility, consult with your medical director to create protocol using the resources below.
  • Share the sepsis process map/workflow with all team members.
  • Add your sepsis process map/workflow onto your storyboard.
