Itemized Invoices Now Available!

Clients may now elect to receive itemized invoices from Purdue University. Reply to this email to opt-in. Please see below for more information and features. By opting-in to the itemized invoices, you will begin receiving a separate invoice for each accession. This differs from the current summarized invoice that will include multiple accessions on a single invoice.

  • The top portion of the invoice will have the header and contact information specific to the ADDL. This will make it easier and faster for you to contact ADDL with billing questions.
  • Clinic information will be displayed on the left.
  • The right column contains familiar information (invoice date, due date, customer number, and amount due).
  • Now the Purchase Order line will display the name of the owner.
  • Please note that there is a character limit and long names will appear shortened.
  • The middle portion will now show itemization of tests completed by ADDL.
  • Each line will be for a separate test.
  • The name of the test, quantity, and fee will be displayed.
  • The top line is for the accessioning fee.
  • It will also show the date the case was received at ADDL, the Accession Number, and the submitting Veterinarian's Name.
  • Please note that there is a character limit and long names will appear shortened.
  • The bottom section will continue to show the amount due and information for mailing checks.
  • If you wish to pay by credit card, please see the website listed in the top header for Online Payment Option. You may also access the link by selecting Online Bill Pay when visiting the ADDL website.
