Community Outreach fulfills the JPS mission of “transforming healthcare delivery for the communities we serve” by strengthening and building community links, internal and external to the network, and improving the health and well-being of the residents in Tarrant County.

Community Outreach Updates

On June 24, JPS participated in the THR Mobile Health Community Resource Day. This event was held to provide resources to those who are experiencing homelessness in the 76104 and 76105 zip codes. JPS Street Medicine team and other departments were on site to provide services and education.

Staying Healthy During the Summer

JPS teams hosted a healthy eating and cooking class on June 29. This class focused on the "MyPlate" concept. It taught participants the importance of incorporating fresh fruits and vegetables into the diet, preparing healthy meals on a budget, and foods that aid in maintaining overall health.

JPS Helps Celebrate Juneteenth

On June 14-15, JPS team members participated in the Empowering You Education, Health, and Job Fair held at Tarrant County College -Trinity River Campus. The Empowering You event was part of the Juneteenth celebration held in Fort Worth. JPS teams provided education on chronic disease and held break-out sessions on suicide prevention.

Master Facility Plan Update

Medical Home Southwest Tarrant

The exterior facade of Medical Home Southwest Tarrant has

been installed along with drywall on the structure's interior. Medical Home Southwest Tarrant is expected to open in Winter 2024.

Community Partnerships

The Tarrant County Food Bank RED Bus will be making a special visit to the Polytechnic Health Center on August 14 to help residents living in the community access SNAP benefits and provide emergency food while supplies last.

Information Corner

JPS Health Network will be participating in the 9th Annual Alzheimer's Education Seminar on August 17. Stop by our table to receive information on JPS Connections and Behavior Health. JPS Geriatrician Dr. Melissa Hudson will be leading a break-out session. This session will provide valuable education for caregivers, families, and healthcare providers.
