Community Outreach Updates


Coming Soon! JPS Health Network, in partnership with the City of Fort Worth's Como Community Center, is implementing a wellness program that will focus on care coordination, health education, navigation and information, and wellness and prevention activities.

Community Outreach Highlights: 2022 PA of the Year


2022 PA of the Year Cares for Patients Experiencing Unsheltered Homelessness

Joel J. Hunt, PA-C, is the 2022 recipient of the PA of the Year Award. A pioneer in street medicine, Hunt works as the Acclaim Director of Street Medicine at the JPS Health Network in Fort Worth, Texas, where he provides primary care to unsheltered and homeless individuals.

Along with the JPS Health Network Board of Managers and Tarrant County Commissioners Court, JPS is currently developing plans to greatly expand our ability to care for the people of Tarrant County. This 10-year Bond Program will expand the services at the JPS Main Campus and its ambulatory network in Tarrant County.

National Minority Mental Health Awareness Month is observed each July to bring awareness to the unique struggles that racial and ethnic minority communities face regarding mental illness in the United States. The COVID-19 pandemic has made it harder for racial and ethnic minority groups to get access to mental health and substance-use treatment services.

Commissioner Roy Charles Brooks cordially invites you to attend the 7th Annual Healthy Lives Matter Alzheimer’s Education Seminar on Saturday, August 13, 2022, from 9 a.m. to 2 p.m. The event will be held at Tarrant County College Trinity River Campus, 300 Trinity Campus Circle, Fort Worth, Texas 76102. On-site registration begins at 8 a.m.

This year the annual Back to School Roundup, will provide free school supplies to 10,000 underserved students in grades Pre-K through 12th who reside in Tarrant County and meet specific income requirements.


This year, the Roundup will offer all of the social services, haircuts, physicals, and other items. Don’t miss this year’s event on Friday, August 5 at Tarrant County College South Campus.

The first day of school is just around the corner and while some students anticipate the start of a new school year, others worry about not having the supplies they need. The 8th annual Back to School Kickoff is back in person at AT&T Stadium on Saturday, August 6 to help with FREE school supplies and services so every kid is ready for the first day of school with confidence.

As part of a COVID-19 Health Disparity Grant, JPS Community Outreach is conducting a voluntary survey to help us improve and reduce COVID-19 health disparities. This survey will help us gather information about ways people reduce the spread of COVID-19.

Please help us preserve our ER and Urgent Care facilities by scheduling an appointment with your provider or by visiting tarrantcounty.com.

If you are a patient at JPS:

JPS patients can use MyChart or call 817-702-1100 to schedule an appointment with their PCP.

If you are NOT a patient at JPS:

Those needing COVID-19 testing or vaccines can visit tarrantcounty.com to find a location nearest you.

The Concilio


The Concilio's mission is to build stronger communities by unlocking opportunities for Latino families. We are an ally and champion for Latino and other underserved minority families in North Texas. We provide direct services and turn-key programs that engage and connect families to resources and opportunities to maximize their quality of life from cradle to career. The Concilio's programs serve over 11,000 families each year to inspire them and equip them with tools that build their confidence to overcome obstacles and unfavorable conditions that interfere with their families' health and education. The Concilio is honored to celebrate over 40 years of serving the DFW community and being recognized as an expert on Latino issues, culture, and parent engagement. Our cultural competency and grassroots service model allow us to gain the trust of traditionally hard-to-reach, Spanish-speaking, low-income populations. The Concilio has served over 250,000 individuals through education and health programming, 76,594 individuals through the 2020 Census outreach program, and an additional 150,000 through the 2021 Vacunas (COVID-19 vaccine) outreach campaign. The Concilio's service model address inequities in Latino health and education through parental and community engagement programming.
