 It is important to understand how the ESRD Quality Incentive Program (ESRD QIP) impacts your facility and your patients. Here are a few resources to help you and your patients learn more about the ESRD QIP:
Emergency/Disaster Reporting Requirements
Reporting Facility Closing/Change in Schedule
Regional, local and facility level events that impact the operation of dialysis facilities or patient access to treatment during normal operating hours must be reported to both the Network and the State Department of Health within 24 hours of the event.  
The Network is required by The Centers for Medicare & Medicaid Services to track the status of facility operations and patient access to treatment during emergency events.  
Reporting Facility Closing/Change in Schedule
REMINDER to Dialysis Providers: You MUST report all interruptions in service to the Network and to the New York State Department of Health
To Report Facility Status to the Network: Reporting Form (via fax or e-mail) or online
To Report Facility Status to the New York State DOH: Enter the information in NYPORTS

The Forum of ESRD Networks' Catheter Reduction Toolkit was developed to offer healthcare providers tools to reduce catheter use, and create solutions to AV fistula and graft barriers to placement. Files in the toolkit include:

NANT, the National Association of Nephrology Technicians/Technologists offers webinars to members and non-members. Participants can earn 1.2 contact hours for EACH webinar.
