Continuing the Fight against Covid-19
Last year brought challenges none of us could have ever imagined. Our thoughts and prayers are with the victims of Covid-19 and their families and loved ones. Now, more than ever, we must remain diligent in our efforts as a community to help stop the spread of the virus.

The 1889 Jefferson Center for Population Health (CPH) has been working with local, regional and state organizations in order to provide education, resources and coordination during this public health crisis. We are asking for your help in sharing this information with your friends, family and co-workers, so we can all enjoy a healthy and happy new year.

If you or your organization are interested in a Winners Wear Masks yard signs created by the Jewish Healthcare Foundation, please email us at CPHinfo@jefferson.edu.
Yours in health,

Community Care HUB
Community Care HUB: By the Numbers
314
Pathways Opened
124
Referrals to local social service agencies
58
Participants served
The Community Care HUB has been in action since September 2020, and HUB Community Health Workers (CHWs) have been hard at work reaching out and making connections alongside 58 high-risk participants since then. Covid-19 has made the launch of a home-visiting initiative difficult to say the least, but CHWs have risen to the challenge. Since September, 314 total pathways have been opened for HUB participants. A pathway is opened when a CHW identifies a barrier to a participant's overall health, such as housing, transportation, social service referral, or food security. There have also been 124 social service pathway referrals made to date, which means that HUB CHWs are busy connecting their participants to the many existing agencies and services in our region. The Work Begins with You:
Michele Moss, CHW
Check out this month’s CHW spotlight with Michele Moss, employed by HUB care coordination agency, Community Action Partnership of Cambria County.

"The first step to understanding HUB participants is acknowledging there is no person the same as another. The world is full of challenges and realizing that every person has their own struggles is the first step. We must understand that a persons’ limitations can be improved, accommodated, and changed completely with the right resources and approaches."

Tammy Rhoades and AUCP:
Rising to the Challenge
This month, we're featuring an interview with Tammy Rhoades, CEO of HUB care coordination agency, Alleghenies United Cerebral Palsy. Check it out!

“The work has continued. We haven’t stopped. But how do you overcome the face-to-face and personal touch interactions when you can only do virtual meetings? We’ve been on-hold here, making phone calls – but unless there is an emergency or crisis situation, everything is done via phone call until we hear about the vaccinations.”

Local Foods, Local Places
Community Action Plan Released
The Community Action Plan for Local Foods, Local Places initiative is now available to view. As part of Vision Together 2025 initiative, the 1889 Jefferson Center for Population Health and several other community partners planned and participated in the Local Foods, Local Places workshop that took place in October. Since then, we've worked with community members to better refine the plan and start implementing the action items.
