Continuing the Fight against Covid-19
Last year brought challenges none of us could have ever imagined. Our thoughts and prayers are with the victims of Covid-19 and their families and loved ones. Now, more than ever, we must remain diligent in our efforts as a community to help stop the spread of the virus. 

The 1889 Jefferson Center for Population Health (CPH) has been working with local, regional and state organizations in order to provide education, resources and coordination during this public health crisis. We are asking for your help in sharing this information with your friends, family and co-workers, so we can all enjoy a healthy and happy new year.

The Community Care HUB has been in action since September 2020, and HUB Community Health Workers (CHWs) have been hard at work reaching out and making connections alongside 58 high-risk participants since then. Covid-19 has made the launch of a home-visiting initiative difficult to say the least, but CHWs have risen to the challenge. Since September, 314 total pathways have been opened for HUB participants. A pathway is opened when a CHW identifies a barrier to a participant's overall health, such as housing, transportation, social service referral, or food security. There have also been 124 social service pathway referrals made to date, which means that HUB CHWs are busy connecting their participants to the many existing agencies and services in our region. Lights of Remembrance:
A Memorial for Cambria COVID Victims
November 19, 5:30-8:30 pm
The Stone Bridge, Johnstown, PA
In This Together Cambria, 1889 Jefferson Center for Population Health, and the Tribune-Democrat are partnering to present Lights of Remembrance: A Memorial for Cambria County COVID Victims from 5:30-8:30 pm on January 19. The memorial, which will be presented at the river wall beneath the Iron Street side of the Stone Bridge, will consist of one candle luminary for each of the 340 people who have died of COVID in Cambria County to date. Local Foods, Local Places
Community Action Plan Released
The Community Action Plan for Local Foods, Local Places initiative is now available to view. As part of Vision Together 2025 initiative, the 1889 Jefferson Center for Population Health and several other community partners planned and participated in the Local Foods, Local Places workshop that took place in October. Since then, we've worked with community members to better refine the plan and start implementing the action items.
CamTran Needs your Input on Bus Services

CamTran has been working to plan the future of our urban and rural bus services in Cambria County. CamTran has developed three options for improving our urban bus services and identified several opportunities for improving our rural services. CamTran needs your help to determine which bus service improvement options work best for Cambria County residents and workers. Provide your feedback today!
