Standard Authorization Form Finalized
Under Ohio Revised Code § 3798.10 , the Ohio Department of Medicaid (ODM) is required to develop a standard authorization form covering the use and disclosure of Protected Health Information (PHI) in alignment with the Health Insurance Portability and Accountability Act (HIPAA) privacy of individually identifiable health information rule 45 C.F.R. § 164.508 and 42 C.F.R. Part 2 regarding the confidentiality of substance use disorder patient records by a Part 2 program.

The purpose of the standard authorization form is to improve care coordination for Ohio patients, including making it more efficient to share PHI in accordance with federal and state requirements and to streamline the integration of physical and behavioral healthcare in Ohio. While Ohio healthcare providers are not required to use the form, they are required to accept and act upon a properly executed form.

ODM has posted information on the Standard Authorization Form in the providers section of their website here: https://medicaid.ohio.gov/provider . Scroll about half way down the page and you will find the “Standard Authorization Form” information which include a link to a fillable PDF version of the form, instructions for completing the form, a fact sheet about the form and the e-mail address of [email protected] where questions about the form may be submitted.

The behavioral health care field, both locally and nationally, is experiencing a workforce shortage.

StarkMHAR has partnered with the University of Akron School of Social Work to create a cohort model for a master’s degree program in Social Work located at the StarkMHAR office in Canton to help address this growing issue.

The MSW Cohort has been postponed until Fall 2019 . For additional questions, please contact Jeannie Cool, Manager of Programs & Evaluation at StarkMHAR.

StarkMHAR Shout-out!
StarkMHAR would like to recognize Angenita Daniels from Coleman. Angenita worked with a client that was having difficulties with stabilization and because of this, was at jeopardy of losing his housing. Angelita was able to appropriately advocate and assist with planning and coordinating in order for the client to maintain stable housing.

Thank you for all that you do!
