What Families Need to Know About the Community Care Program (CCP) and Waiting List

The Community Care Program (CCP) is a Medicaid waiver program that provides vital services to individuals eligible for Division of Developmental Disabilities (DDD) services. These services help individuals live in their own homes, with family, or in licensed residential settings while avoiding institutionalization.

What is the CCP?


The CCP offers services funded by a combination of state and federal Medicaid funds to help individuals with intellectual and developmental disabilities live safely and independently in the community.

Key Points About the CCP:


  • No Double Enrollment: Medicaid prohibits enrollment in more than one waiver program. A person enrolled in CCP cannot also be enrolled in the Supports Program (SP) or Managed Long-Term Services and Supports (MLTSS).
  • Who It’s For: CCP is for individuals with more significant needs that cannot be met through the SP or other programs

Who is Eligible for the CCP?


To qualify for CCP, an individual must meet all of the following criteria:

  1. General Requirements:
  • Eligible for DDD services.
  • Maintain Medicaid/CCP financial eligibility.
  • Meet the Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/ID) Level of Care (LOC) standards.


2.Specific Circumstances:

  • On the CCP Waiting List: Individuals are added to the list chronologically and must meet LOC criteria when reached.
  • Emergency Situations: Immediate CCP access may be granted for homelessness or imminent peril that cannot be resolved through the SP.

What is the ICF/ID Level of Care (LOC)?


The Level of Care (LOC) refers to the level of support needed for an individual to ensure their health and safety. To meet CCP LOC requirements, individuals must have substantial limitations in at least three functional areas (one must be self-care) and meet these criteria:

  • Have a developmental disability diagnosis.
  • Need continuous, active treatment related to the disability.
  • Require a 24-hour plan of care and intensive training.
  • Would require institutionalization if not for CCP services.
  • Cannot be safely supported in the SP.

How to Join the CCP Waiting List


To request to be added to the waiting list:

  1. Complete the Form: Fill out the Community Care Program Waiting List Request Form
  2. Submit the Form:

Emergency Enrollment in CCP

In emergencies, such as homelessness or imminent peril, CCP enrollment may be expedited. Here’s how to request consideration:

  1. Notify your Support Coordinator (SC) about your situation.
  2. The SC will work with their supervisor to submit an Intensive Case Management (ICM) Referral with detailed information.
  3. The DDD Intensive Case Management Unit may conduct an in-person visit to gather more information.

Examples of Emergency Situations:

  • Homelessness: No place to live, or current living arrangements will end within 30 days.
  • Imminent Peril: Serious health or safety risks that cannot be resolved with additional SP supports.


Other Housing and Service Options

For individuals not requiring 24/7 support, consider these alternatives:

  • Housing Subsidy: Through the Supportive Housing Connection, paired with SP services.
  • Boarding Home: Includes food, shelter, and basic support.
  • Residential Healthcare Facility: Offers meals, housekeeping, personal support services, and nurse oversight.
