Supporting Community-Dwelling Older Adults
AIHS provides both short-term and long-term Case Management services. Our Case Management department promotes aging-in-place by delaying nursing-home admissions and keeping individuals independent in their own homes for as long as possible.
Person-Centered Care and Planning
AIHS uses a person-centered approach within all our services. For Case Management, this approach allows our clients to be integral in all care planning decisions and outcomes.

What the person-centered approach looks like:

  • Find out what is most important to the client
  • Support client to have positive control, including health and safety over the life they desire
  • Recognize that supports are designed in the way the client wants them
  • Allows the client to advocate and speak for themselves, even if they need a little help
AIHS was the first Area Agency on Aging/Community-Based Organization to receive
NCQA Accreditation for Long-Term Supports and Services Case Management in 2017.


NCQA Case Management Accreditation addresses delivery of case management services, not just the organization’s internal processes. The quality-improvement framework includes meeting standards of using and developing personalized, person-centered assessments and care plans. AIHS received renewal of this achievement in 2020.
Case Management Success Stories
Client’s spouse “Arlene” joined AIHS’ Alzheimer’s Disease Program Initiative (ADPI) to receive education about dementia and tips to cope with it and behavioral disturbances. After just 3 months, caregiver’s HABC (Healthy Aging Brain Center) score went from 16 to 6 thanks to Adult Day Care and in-home services for her care recipient, and attendance at Family Caregiver Support Groups.

“Ralph” was referred to AIHS wanting to discharge from a Nursing Facility where he had been admitted for failure to thrive at home alone. AIHS set up in-home supports and therapy services. In just 4 months, he no longer uses a wheelchair and can ambulate on his own using just a walker.

“Marie” has multiple chronic conditions resulting in the recent amputation of her leg below the knee. Her daughter is her caregiver and can now be paid through the Structured Family Care program. Client is able to live at home thanks to home-delivered meals, a personal emergency response system, and several home modifications making her home safe and handicap accessible.

The City of Fort Wayne Needs Your Input
The City of Fort Wayne’s Office of Housing & Neighborhood Services has received more than $4 million in federal funding to address homelessness. Now, the City, in collaboration with the nationally-recognized consulting firm Homebase, is seeking public input on the best way for the City to spend those dollars.
