Reminder: Misrouted results and documents

The OPOR team is aware of ongoing routing challenges that may result in reports and results being sent to the CIS instead of an EMR. We are actively implementing mitigations, including enhanced education and guidance for frontline registration staff.

Physicians practicing at multiple locations—particularly those working in both the CIS and an EMR—may continue to experience results or documents being routed to the wrong practice location.

To help ensure results are routed correctly, please include the complete practice name, address and external provider ID on all outside requisitions. This information helps registration staff identify the correct provider and practice location during patient registration, reducing the risk of misrouted results. If you encounter a routing issue, please report it using the process outlined here.

Support Better Access to Long-Term Care Through Timely Completion of Medical Status Reports

The Nova Scotia Health Continuing Care Medical Status Report (MSR) provides medical information concerning a patient when admission to a long-term care (LTC) facility is considered. As a required part of the LTC application process in Nova Scotia, this form communicates a healthcare provider's assessment that a patient is medically suitable for admission to a LTC facility.

Continuing Care relies on the completed MSR to support timely transition for individuals who can no longer be supported in the community. When MSRs are delayed, clients will wait longer to transition to LTC, and a LTC bed will remain vacant. Time spent by Continuing Care making repeated calls, sending reminders and seeking clarification on outstanding reports reduces the time available to support other clients and families in the community who require assessment, care planning and coordination of services.

How healthcare providers can help:

  • Complete and submit MSRs as soon as possible following receipt of a request from Continuing Care or a patient/family.
  • Ensure all sections of the form are fully completed to avoid the need for follow-up, including signing the medication list on each page.
  • Recognize the important role the MSR plays in enabling timely access to LTC and supporting effective care coordination.

Your partnership with Continuing Care is essential in helping Nova Scotians access the care they need, when they need it. Timely completion of the MSR supports efficient decision-making, reduces administrative burden for all involved, and, most importantly, helps patients and families navigate the transition to LTC with fewer delays.

Thank you for your continued collaboration and commitment to providing high-quality, patient-centred care.
