New and Upcoming HMIS Updates

HMIS Training now in ATP!

We have been working closely with the Close to Home team to prepare to reintegrate HMIS training into the Alliance Training Portal (ATP). We are excited to announce that all HMIS Training has transitioned to ATP as of Tuesday, July 1, 2025.


All training requests received from this point will be fulfilled using the HMIS Learning Pathways in ATP. The process for requesting HMIS Training will remain the same.


We will continue to collaborate with CTH and supervisors to ensure the transition is a success and users are able to complete the HMIS Training in a timely manner. We are excited for the opportunity to advance our training procedures and welcome any feedback on the process to help improve our processes.

Prevention Income and Eligibility Updates

The Prevention and Housing Subcommittee worked diligently to propose updates to the Homelink Prevention CE eligibility requirements to ensure our community is able to swiftly and efficiently serve individuals at risk of losing their housing.


These updates include limiting eligibility to individuals 30% or below AMI due to our funding requirements, and utilizing the HUD Financial Assessment in the workflow to accurately capture all types of income that need to be considered when determining eligibility.


These updates went live in HMIS on Tuesday, July 1st. The Prevention Reports, Prevention Resource Eligibility List & Prevention Project Monitoring List, have been updated to reflect the new eligibility requirements, and clients enrolled prior to this date do not need to be reassessed. The previous versions of the reports are available for historical reporting as needed.

New Assessment coming July 21st!

Starting Monday, July 21st, all projects populating the Priority Pool (PP) and Resource Eligibility List (REL) will have a new assessment page added to the Entry Workflow. This assessment, titled the Physical Needs and Medical Equipment Survey, is intended to help our community gather information on the medical needs of clients.


The survey will include 7 questions for adults, and 2 for any children in the household. Questions will include:



  • Are you able to dress yourselves without assistance?
  • Do you use or need a mobility device? (wheelchair, walker, etc.)
  • Do you use or need Durable Medical Equipment? (CPAP machine, oxygen concentrator, dialysis machine, etc.)


All questions will also include a section allowing the assessor to Agree or Disagree with the client's answers based on observation and client's known history.


These questions will not be used to determine eligibility for any projects. The survey will be used to address a significant gap in data regarding clients who require a higher level of care. The data will help our community identify need for care and inform future resource planning and service enhancements, specifically for potentially developing or utilizing medical respite or high-level care facilities.


Training on conducting this survey will be held by Close to Home in mid-July before the launch date of the survey.
