Innovative VA Concurrent Care Program available in Hawai‘i

At our May K ōkua Mau meeting, Dr. Liane Tokushige presented about the innovative and timely palliative care program bringing in-home care to veterans with serious illness. Here is a description of the program:

VA PIHCS Concurrent Care program is for registered Veterans with advanced illness such as cancer, heart failure or advanced lung disease to be able to access by phone or in their home, medical care focused on alleviating symptoms such as pain, nausea, or anxiety, 24 hours a day, while still receiving treatments. Under this program, the Veteran may continue to visit their doctors, receive treatments such as chemotherapy and visit the emergency room if needed. The actual hands-on care is provided by community hospice agencies, statewide. If interested, call your VA provider to request a palliative care consult.

Advance Care Planning Best Practices Report with focus on Hawaii
C-TAC has released  the full version of a report  designed to identify strategies that could be used to boost the use of advance care planning (ACP), a practice that is now more important than ever as we face the uncertainties of the COVID-19 pandemic.  Discern Health , a healthcare quality and research advisory organization, developed the report. 

Hawaii features prominently in the report as we are leading the nation in Advance Care Planning in certain measures, although there is clearly a long way to go. Jeannette Koijane, Hope Young, Dr. Rae Seitz and Dr. Emese Somogyi were interviewed along with Dr. Angelo Volandes from ACP Decisions.

Thoughtful conversations about goals of care are more important now than ever before as we face the challenges of the COVID-19 pandemic ,” said C-TAC Executive Director Jon Broyles. “ This report identifies several key practices that can be used to promote advance care planning across the country and ensure that people receive the care they want in the setting that is right for them .”
Fillable/writable POLST form available
Based on community feedback, we created a fillable/writable PDF file for the official POLST form for Hawai‘i. Thank you for this great tip! As far as we know, we might be the first state offering a fillable PDF file for our POLST.

We hope it makes your work easier. You can download it on our POLST for Providers page. All appropriate fields can be filled out with word or tick marks. Keep in mind to only choose one option per section. The signature fields cannot be filled out electronically, since POLST needs to be signed in person the patient and an APRN or MD.

Please download the file ‘ Hawaii_POLST_Form_fillable.pdf’. Fill it out in your computer or tablet, print it and sign it. Green paper is strongly recommended. Please note: This PDF file is not a fillable web form or an electronic record and consequently all information you put in inside your browser might be lost.

People living with serious illness are at high risk for COVID-19. This is the population treated by palliative care. So many patients and caregivers are wondering what else can we do aside from washing hands, social distancing, and staying at home?

We are all in this together. You can do your part by making a plan. This plan can help you, your family, friends, and your medical providers.
Reimagine: Life, Loss, & Love Virtual Festival 5/1 to 7/9
Reimagine: Life, Loss, & Love” is a worldwide virtual festival which runs through July 9th.

Featuring numerous “ changemakers” from healthcare, end-of-life care, the arts, and spirituality, the festival is focused on “embracing life, facing death, and loving fully in the face of COVID-19.”

Workshops, concerts, conversations, ceremonies, book club discussions, film screenings, games, dances, dinners, demonstrations and more center around the themes of “wonder,” “prepare,” “remember,” and “live fully.” Organizations such as Death Café, Compassion and Choices, End of Life Choices New York, Steady Waves End of Life Services, and many others are holding special presentations and events. The ELNEC team (End of Life Nursing Education Consortium) extends their support for all clinicians providing palliative care and hopes you make sure to take care of yourselves during this challenging time. During the COVID crisis Coalition for Compassionate Care of California is making free digital versions of their popular Decision Aids available to download and share with consumers (though May 30, 2020).

CCCC’s Decision Aids explain the complex topics of life-sustaining treatments using consumer-friendly language with neutral, evidence-based information.
