'Ask A Pemi-Baker Nurse': A collaboration with the Grafton County Senior Citizens Council (GCSCC)

If you have questions or need a place to cool off why not visit your local senior center? In addition to virtual and in-person activities and meals offered by the 8 senior centers of GCSCC, five are hosting Pemi-Baker Hospice and Home Health’s ‘Ask A Pemi-Baker Nurse’ program. This is a free service manned by a Pemi-Baker Registered Nurse and/or Social Worker where you can get your blood pressure checked, ask medical questions, fill our your advance directive forms or just swing by and say hi!



Ask-A-Nurse Days & Times:

Plymouth Regional Senior Center-GCSCC, 8 Depot Street, Plymouth: Every Monday except holidays 11:30am-12:30pm

Orford Senior Center-GCSCC, Dartmouth College Highway, Orford: First Tuesday every month, 10am-11:00am

Littleton Senior Center-GCSCC, 77 Riverglen Lane, Littleton: 3rd Tuesday every month 11:30am-12:30pm

Horse Meadow Senior Center-GCSCC, 91 Horse Meadow Road, North Haverhill: 2nd Wednesday every month 10:30-11:30am

Lincoln/Lin-wood Senior Center-GCSCC, 194 Pollard Rd, Lincoln: Fourth Wednesday every month, 11:00am-12:00pm

Goals of Care Conversations

People often come to a crossroad in their medical care when the time comes to make some difficult decisions. Perhaps they received a new diagnosis, are considering treatment options or have excessive burden from their current medical treatment. Our team often facilitates these important conversations with patients, their care givers or family members, so that a person’s medical care is consistent with their true goals and values. However, these conversations can be had with your provider and within your family at any time. The Conversation Project, theconversationproject.org, is a resource anyone may use to facilitate these conversations and help you consider what matters most.

With the increasing age of our population, many people are finding themselves providing care for those in need. Connect with others as we talk about monthly caregiving topics and the ups and downs of at home caregiving. It's important to have the tools and resources you need to cope with the natural ebb and flow of emotions. First Wednesday of every Month: July 3rd, 10am!
