Things to Know

Volusia & Flagler CHNA Survey is Open


As part of the 2024 Flagler County and Volusia County Collaborative Community Health Needs Assessment (CHNA), Flagler and Volusia County residents 18 years and older are encouraged to fill out a short survey to share their opinions about healthcare and the quality of life in their county. The survey will be open through July 31, 2024. Please forward the survey to your contacts!

Resource Development for RCORP-NAS


Our staff is growing the consortium behind the RCORP-NAS grant project in Nassau County. We are contacting potential partners through pamphlets, letters, emails, phone calls, and on-site visits. If your organization provides wrap-around services in Nassau County or if you know of one that does, contact Erin Patrick at Erin_Patrick@hpcnef.org or at 315-717-5464.

Partner Spotlight: PALS SkyHope


PALS SkyHope is a nonprofit that provides free medical flights (by volunteer pilots) to and from treatment for those who may be too far to travel by car. They serve all areas east of the Mississippi River! Refer patients to the request form on their website.
