Community Bridges is a non-profit healthcare provider offering medically integrated behavioral health services since 1982. We operate innovative models of care across inpatient and outpatient, crisis, peer support, housing, prevention, and treatment services. CBI is one of the largest statewide providers in 10 of Arizona's 15 counties, including Maricopa, Pima, and Pinal counties. Our dedicated team of medical, behavioral health, and peer professionals are guided by CBI’s mission to maintain the dignity of human life.
Together with HSC, we offer Phoenix Rise and PATH to help individuals rebuild their lives. Phoenix Rise provides behavioral health services for individuals experiencing homelessness on the Human Services Campus. Phoenix Rise offers an outpatient treatment setting which supports individuals as they make progress toward long-term health.
Projects for Assistance in Transition from Homelessness (PATH) is a street outreach program that serves Maricopa County. It is designed to outreach and engage individuals experiencing homelessness who have underlying mental health conditions.
