Case of the Month
Managing a Privacy Breach
Manuel is an occupational therapist in an outpatient clinic. He recently recommended adaptive equipment to his client and helped them apply for funding. When submitting the funding application form, he copied and pasted excerpts of the client’s information from the clinical record into the application.

Weeks later, the funding organization contacts Manuel and tells him there was personal health information related to another client on the form. He realizes that he copied and pasted information of another client into the form.

This inadvertent disclosure of an individual’s personal information is a privacy breach. Manuel is wondering what he should do.
