As of June 14th, CHDC is offering the Pfizer COVID-19 vaccine for children ages 12-17.
Protect yourself and others against Covid-19. Please continue to follow guidelines, wash your hands, practice social distancing, and get vaccinated.

Happy Summertime!
We are all eager for some great weather and fun in the sun this summer! CHDC has been very busy the past few months as we prepare to bring back our services to full capacity. With about 45% of Americans vaccinated, we still urge each and everyone of you to get the COVID vaccine as the last thing we want is to continue limiting services and face shutdowns in the fall/winter. We are looking to partner with community organizations in our service area as soon as the FDA approves the COVID vaccine for children 12 and younger. I am also very happy and proud to announce CHDC received a national prize award to collaborate with the Pottstown YWCA and Rickett’s Center to increase access for children to receive their age-appropriate vaccinations and provide Centering Well- Child checks at these locations.
Please keep a look out for our new dental mobile unit which will be placed at our Barto, PA location in July for a few weeks to provide dental services to this community. Do not forget, if you or someone you know is uninsured and/or underinsured we urge you to apply for a Healthcare discount; you could pay as low as $35 per dental office visit!
We are also waiting on our new mobile medical unit which should be delivered in the fall of 2021, as long as supply shortages do not get in the way! This mobile medical unit will allow CHDC to provide services at schools, churches, nursing homes, colleges, etc. so if anyone has a need, please email us at info@ch-dc.org and we can discuss if our medical mobile unit can work for your organization needs. Remember, we offer medical, dental, physical therapy, optometry, OBGYN, behavioral health, MAT, care and case management, medication dispensary, free transportation and much more!

Community Health & Dental Care, Inc. Among Winners of P4Challenge - Awarded for Innovation in Promoting Pediatric Primary Prevention to Improve Child Health
We are honored to have been selected among 50 national winners in the Health Resources & Services Administration (HRSA) Phase 1 P4 Challenge for innovation in promoting pediatric primary care to improve child health. The goal of the nationwide P4 Challenge is to increase access of well-child visits and improve immunizations within pediatric primary care settings.

Community Health Workers Assisting Pottstown Street Medicine Program
Every Tuesday, our Community Health Worker’s (CHW’S) Gladys Rodriquez and Camille Watson join other local health care professionals in Pottstown for the Street Medicine Program. They start their day meeting in the community, doing a “huddle” to discuss at risk patients they hope to encounter and offer care, including recent hospital discharges. The CHWs register/schedule new patients, connect them care in the community, and offer information about the many CHDC services. The Street Medicine Team check vitals, treat wounds, and even prescribes and deliver meds that day. They follow up with patients discharged from the hospital.
Other community partners that support the Street Medicine program include Pottstown Pharmacy, Access Services, Pottstown Hospital-Tower Health and Creative Health Services. After 2.5 months of participating in the program: 25% of the patients who connect with our Street Medicine team end up becoming patients. 17% have had more than one CHDC appointment.

Chester County Food Bank Fresh2You Mobile Market at CHDC
The Fresh2You Mobile Market is open at its new location, the CHDC parking lot at our Coventry Mall location, on Tuesdays from 2:30pm- 4:30pm. They will be bringing us fresh, delicious, seasonal fruits and vegetables and even offer cooking demonstrations and prepackaged dry goods such as grain bowls, seasonings, and beans.
Fresh2You accepts all forms of payment, including cash, credit/debit, SNAP/EBT and WIC or Senior Farmer’s Market Nutrition Program (FMNP) checks. For customers shopping with SNAP/EBT or FMNP, Fresh2You can stretch your food budget by offering matching dollars in the form of Veggie Bucks.

CHDC Mobile COVID-19 Testing & Vaccination Clinic Hits The Road in June
No Appointment Required!
CHDC will be performing COVID testing and administering COVID vaccines via our shuttle. Individuals 18 years and older are eligible to receive the vaccine. There is no cost to receive a test or vaccine.
Our shuttle schedule is as follows:
- Thursday, July 8
- Monday, July 19
- Monday, August 2
- Monday, August 16
Look for us at the following times and locations (on all scheduled dates):
- 9AM - 10AM, Pottstown YMCA
- 11AM - 12PM, Spring City Public Library
- 1PM - 2PM, Boyertown YMCA

CHDC Expands Dispensary Services with Pottstown Pharmacy Partnership
Community Health & Dental Care, Inc. and Pottstown Pharmacy have entered into a relationship to expand pharmacy services and medication discounts to patients in the Pottstown and surrounding communities.
CHDC patients can conveniently get their prescriptions filled either at the CHDC dispensary at the Coventry Mall, 351 W. Schuylkill Road, Pottstown 19465 or at Pottstown Pharmacy, 1501 East High Street, Pottstown 19464.
Pottstown Pharmacy also offers FREE DELIVERY service within our service area. Eligible patients who need access to medication at an affordable price- especially higher costs meds – can receive medication discounts. Eligible patients must be an established medical patient of CHDC and complete the required application to determine healthcare discount to be applied. Eligible patients applying for CHDC’s healthcare discount program may receive medications for as low as $5.00 a prescription. Referrals welcome.

COVID-19 Health & Safety Precautions

At Community Health and Dental Care, we are committed to a safe and clean environment as the COVID-19 pandemic continues. Locations have been triple-cleaned, all staff are wearing masks, and seating in lobby and waiting rooms are socially distant.

Octodent equipment have been installed in our dental facilities, and patients are being scheduled to allow for appropriate levels of social distancing in our facility. COVID-19 pre-screening includes temperature checks of all patients upon arrival for their scheduled appointment. All patients and staff are required to wear masks.
