KDIGO Publishes CKD-MBD Controversies Conference Report

KDIGO has published the final report from its Controversies Conference on Chronic Kidney Disease–Mineral and Bone Disorder (CKD-MBD): Progress and Knowledge Gaps Toward Personalizing Care, co-chaired by Markus Ketteler, MD (Germany), and Rosa Moysés, MD (Brazil). Published in the March issue of Kidney International, the report highlights the urgent need for a patient-centered approach to CKD-MBD management, outlines key knowledge gaps, and calls for further research.


“Our understanding of CKD-MBD has grown far beyond simply managing biochemical markers,” said Dr. Ketteler. “This report highlights the intricate relationship between bone health and cardiovascular disease in CKD patients and calls for a personalized care model that addresses the broader clinical picture. Managing CKD-MBD isn’t just about correcting lab values—it’s about preventing fractures, mitigating cardiovascular risks, and improving quality of life. It’s time to move away from rigid, one-size-fits-all frameworks and towards strategies that reflect the unique needs of each patient.”  


The report introduces a framework that categorizes CKD-MBD into CKD-associated osteoporosis, focusing on fracture prevention and bone health, and CKD-associated cardiovascular disease, addressing vascular calcification and cardiac complications. It calls for advanced imaging, novel biomarkers, and individualized phosphate and calcium management to balance bone and cardiovascular health. The report also emphasizes refining treatment targets for parathyroid hormone (PTH), vitamin D, and other mineral metabolism markers, while advocating for more inclusive clinical trials, particularly for post-transplant and pediatric populations. 



“One of the most powerful messages from this meeting is that CKD-MBD management must center on the patient, not just the disease,” said Dr. Moysés. “This report recognizes that factors like inflammation, uremic toxins, immune dysregulation, and gut health all influence CKD-MBD progression. We need strategies that go beyond surface-level treatment and address the root causes, while also considering the patient’s overall health, lifestyle, and preferences. By embracing a holistic, patient-centered approach, we can reduce fracture risk, slow cardiovascular disease progression, and ultimately improve survival and quality of life.” Read the full report.

KDIGO has produced a new video series highlighting key takeaways from the KDIGO 2025 Clinical Practice Guideline for the Evaluation, Management, and Treatment of Autosomal Dominant Polycystic Kidney Disease (ADPKD). The series is designed as a practical resource to ensure that the latest KDIGO recommendations are accessible and applicable in real-world clinical settings.


The videos feature insights from ADPKD Guideline Work Group Members Arlene Chapman, MD (United States), and Peter Harris, MD (United States), along with Guideline Co-Chair Vicente Torres, MD (United States). Together, they provide expert analysis of the latest evidence-based recommendations to help clinicians, patients, and caregivers navigate ADPKD care. They cover critical topics such as the classification and diagnosis of ADPKD, disease progression and management strategies, and treatment options, including both pharmacologic and non-pharmacologic approaches. The series also examines extrarenal manifestations of the disease, lifestyle considerations such as diet and exercise, and key issues related to reproductive health and pediatric care.

KDIGO recently published the first of two summit reports from the KDIGO Implementation Summit on Diabetes Management in CKD: From Primary to Collaborative Care in Lancet Regional Health – Western Pacific. The report summarizes discussions from the July 2023 summit in Hong Kong, co-chaired by Juliana Chan, MD (Hong Kong), and Philip Li, MD (Hong Kong). Experts in nephrology, endocrinology, and primary care gathered to address barriers to implementing guideline-based diabetes and CKD care, with a particular focus on strengthening primary care’s role in disease management.


This summit analyzed, and the report highlights, Hong Kong’s success in data-driven diabetes management through initiatives like the Hong Kong Diabetes Register, the Joint Asia Diabetes Evaluation Programme, and the Risk Assessment and Management Programme. These programs have significantly reduced diabetes-related complications and mortality by integrating structured risk assessment, digital health tools, and multidisciplinary collaboration. The report also explores strategies to improve communication between specialists and primary care providers, leverage digital innovations, and address gaps in care for young-onset diabetes, a growing population with a high risk of complications.
