Clinical Update from KHCA Consultant Linda Farrar


Since April 28, 2025 F628 (Transfers & Discharges) has been cited 24 times! And that really only includes 4 months. Actualized out, that means F628 would rival F880 Infection Prevention & Control for a top deficiency citation. In many, but not all, the findings for F628 include the lack of a Discharge Summary at all or lack of a comprehensive Discharge Summary. So this week, this discussion will include what should be included in a compliant Discharge Summary, understanding that many facilities are using a form accessible in the electronic medical record system used by the facility. But knowing that does not always mean staff complete the forms provided.

Final MDS 3.0 RAI User’s Manual Available, Will Be Effective October 1


Provider Magazine (9/8, 151K) reported, "The final Minimum Data Set (MDS) 3.0 Resident Assessment Instrument (RAI) User’s Manual version (v)1.20.1 is now available in the Downloads section on the Minimum Data Set (MDS) 3.0 Resident Assessment Instrument (RAI) Manual page" and "will be effective beginning October 1, 2025." Provider Magazine highlighted updates set to take effect, noting "various minor alignments, clarifications, and corrections have been implemented throughout." 

2026 Webinar Survey


Proactive is currently collecting data on topics for 2026 webinars. KHCA/KCAL is seeking your input on desired topics for next year. The purpose of this survey is solely to see what our members are interested in and is not a complete list of trainings that Proactive plans to provide.  The KHCA/KCAL Education committee will review results and make decisions on education delivered based on your input.

CMS updates timing for Care Compare, schizophrenia changes


The Centers for Medicare & Medicaid Services has updated two key timelines for public reporting changes that will affect nursing homes, including delaying a new quality measure calculation.


While most data on the Nursing Home Care Compare website remains frozen until late October, a CMS memo issued Wednesday clarified that information gleaned from payroll-based journal and schizophrenia coding audits will be updated Sept. 24. Special Focus Facility status also will be updated at that time. READ MORE
