Greetings Kane County IRIS Users!


The Kane County IRIS community convened on May 20, 2024, for our third annual meeting. Meeting Highlights 


Annual Data Presentation (3.15.23 - 3.15.24)

Key highlights from the IRIS Annual Data presentation:

  • We had 8,466 referrals in one year, and 75% (6,418) of those referral loops were completed!
  • 41% of the 6,418 completed referral loops ended in service enrollment.
  • Consistent increase in referrals for each quarter
  • The average response time remained below 5 days, with an impressive average response time of 3.3 this quarter.
  • Sectors least represented in IRIS: Aging Care, Concrete Supports (housing & utility), and Developmental Disabilities.

Strengths:

  • Partners seek to connect clients to additional services.
  • Clients are empowered to choose the services they receive
  • Eligibility and intake procedures are clear
  • Referral tracking

Challenges:

  • Partners having shared expectations around the referral process
  • Services are available when clients need them

Quality Improvement & Goal Setting

During the meeting, the group utilized a Jamboard to discuss quality improvement and goal setting. Various quality improvement areas were mentioned regarding the IRIS system, including but not limited to automatic referral reminders, ensuring organizations offer the requested services, the ability to edit completed referrals, and more.


As Year 3 of IRIS wraps up, goals for the next year were discussed. Some goals mentioned include onboarding more schools, senior services, and developmental disabilities care; utilizing the notes section and communication center in IRIS, and promoting the expansion of the Social Determinants of Health screening tool.

Our IRIS community now includes 197 partners representing multiple sectors, including behavioral health, primary care, dental, parenting resources, substance use services, basic needs (e.g. food, clothing, and emergency shelter), senior services, juvenile and adult probation, early childhood, K-12 education, youth support, veteran services and more.
