Please hold the date for an upcoming PCN Network Event on Thursday, June 18, from 3:00 PM to 8:00 PM at the Fenelon Falls Community Centre. Dinner will be provided.
Registration details will be shared shortly.
This session will focus on practical supports for providers, including tools, tips, programs, and resources to enhance day-to-day practice. It will also offer an opportunity to connect with colleagues and contribute to a shared vision for a more connected and sustainable primary care system across the KLH-OHT.

What Is It?
CSS Central Intake is a single-entry point where you can refer patients to a range of community programs—all in one place.
What Your Patients Get Access To
Through Central Intake, patients may be connected to the full Community Support Services (CSS) basket of services, including:
- Exercise, wellness, and falls prevention programs
- Nutrition supports and meal services (e.g., Meals on Wheels)
- Caregiver support and respite options
- Social and recreational programs (e.g., Adult Day Programs)
- Transportation services
- Assisted living and supportive housing supports
- In-home and community-based supports that promote independence
- And more, based on individual needs
How It Works
- Submit one referral through the Ocean platform.
- Central Intake coordinates with appropriate service providers.
- Patients receive support tailored to their needs.
| | Expanding Access to Primary Care in Our Communities | | |
Over the past year, our region has secured significant new funding through two rounds of the Interprofessional Primary Care Team (IPCT) initiative—$3.4 million in Round 1 and $1.8 million in Round 2.
These were highly collaborative efforts, led by the Kawartha North Family Health Team (Round 1) and the City of Kawartha Lakes Family Health Team (Round 2), on behalf of Ontario Health Team partners.
This funding is helping to grow local care teams, add new health care providers, and connect more people to primary care closer to home. We are already seeing strong progress, with hundreds of residents newly attached to a family doctor or nurse practitioner, and more to come.
This is a clear example of what we can achieve by working together to improve access to care across our communities.
| | Cybersecurity and Phishing Awareness Training | | |
The OH East Digital Health team, in partnership with Infosec, is providing cybersecurity and phishing awareness training licenses for one year.
The goal of this training program is to provide valuable skills for how to protect patient health information, phishing awareness, and mitigate risk at the workplace.
The training is a total of approximately 40 minutes of content including assessments to be completed by January 30, 2027, with the cost of the licenses covered by OH. This training program also includes a phishing email simulation to reinforce learning. The program aligns with the IPC Privacy Management Handbook for Small Health Care Organizations and is designed to be practical and feasible for busy clinical environments.

| | Mental Health and Addiction Resources | | Access Mental Health and Addictions (AccessMHA) | |
AccessMHA makes it easier to help find mental health, substance use health, and addictions support, services, and care.
AccessMHA is a coordinated point of service access for your patient’s mental health, substance use health and addiction needs. A referral to AccessMHA provides access to a suite of services, removing the guesswork of knowing where to refer your patient. A staff member will assess your patient’s situation and will match them to appropriate services. If an appropriate service does not exist within the network, the AccessMHA staff will help connect them with other local services that might be a better fit.
To refer your patient, please fill out the AccessMHA referral form through Ocean Health Map.

- Supports for OAB promotion (posters, post cards, email blasts)
- Reviewing existing OAB offerings and appointment availability
