A life care management assessment has key areas to address to ensure optimal quality of life. Care managers may have different questions under each category based on their specific area of expertise or niche area. The care manager may create their own questions or use evidence-based tools such as a cognitive screen, or the Katz Activity of Daily Living (ADL) screen. Using an organized process or consistent method helps the care manager collect the important and comprehensive information about the given situation.
Although it is important to have pre-set questions, it is also critical to ask more detailed questions depending on the client. All clients are unique, and sometimes complex, and may have special circumstances. Digging deeper helps create a person-family centered and individualized care plan with the recommendations which are best suited to the particular situation.
