Happy summer Incoming Kindergarten Families!
I hope things are off to a great start, and that you are having tons of fun! Everyone at Dorchester is excited for your arrival in September, and we can’t wait to see the great things your child does this coming year.
As we prepare for the 2022-23 school year, we are compiling our students' health files. As noted on the Dorchester Kindergarten Registration page, if you have not already done so, just a quick reminder that the following medical documents are required for the start of kindergarten (Please have everything submitted to me via email by July 5, 2022):
Up-to-date Physical Exam (done within 365 days of the first day of school) If your doctor does not have a form for annual physicals, you may provide them with the Universal Health form linked here.
Complete Immunization Record (this should include 5 doses of DTaP, 4 doses of IPV/Polio, 3 doses of HepB, 2 doses of MMR, 1 dose of Varicella)
Medication Orders from the doctor (if needed while at school, includes any OTC medications)
FARE Allergy Action Plan for any students with allergies, filled out by doctor
Asthma Action Plan for any students with asthma, filled out by doctor
*A reminder that we do not stock any medications for use at school. In the event your child
needs a medication, it will have to be provided from home and be accompanied by a
doctor’s note (even for over-the-counter medications like Advil, Benadryl, eye drops, etc.).
