Schedule Back-to-School Physicals
Our schedules are open through the fall. Don't forget to schedule your back-to-school physical.

Health Form Requests
Due to COVID-19, many families have fallen behind in their well-child care visits - causing an unusually high demand for both visits and various health forms.

Please know our staff and providers are working tirelessly to meet the needs of our patients and families during this time. We are committed to completing health forms in an accurate and timely way.

Forms will be completed and returned in the order that they are received within 10 business days of your request. Please plan accordingly.

Welcome Dr. Jaclyn Winikoff
Dr. Jaclyn Winikoff joined Lurie Children's Primary Care - Town & Country Pediatrics in 2021. She graduated from Georgetown University School of Medicine in 2010 and completed her residency in pediatrics at Children's Memorial Hospital/Ann & Robert H. Lurie Children's Hospital of Chicago in 2013. Prior to joining LCPC-TCP she was working as a hospitalist at Lurie Children's and Prentice for the past 8 years.
Fun Facts about Dr. Winikoff
  • Dr. Winikoff was born in a small town in northern New Jersey and is a third generation pediatrician!
  • She was entering her third year of residency when the hospital moved from Lincoln Park to Streeterville--she was in the PICU holding down the fort at Children's Memorial Hospital during the move, helping load each patient into their ambulance for the drive down Lake Shore.
  • After 10 years in the city, she and her family now live in the northern suburbs of Chicago. She lives with her husband, Ben, and their four girls: two humans named Eliza and Sadie and two fur babies, black labs Georgie and Rosie.
  • Before COVID, there is nothing she liked more than trying all of the amazing restaurants in Chicago.
  • Since the pandemic, her spare time is filled with long dog walks, at-home dance parties with (bad) karaoke and epic games of Uno and Memory Match.
  • She cannot wait to get to know more of the Town & Country Glenview families!
