LGMS ATHLETIC REGISTRATION FORM

For our athletics, we do have forms that are required for participation. We will be having parents/guardians complete the concussion/cardiac arrest agreement (waiver) through google form (linked below). This form is needed for your child to participate in any athletic intramural or interscholastic sport at LGMS but only needs to be filled out once per year. This form also needs to be filled out with your child. We want athletes to know and understand the signs and symptoms of a concussion or possible cardiac arrest. All of the information needed is linked on the google form and this form should take less than 10 minutes to complete.

If you filled this out already, you do not have to do it again.

We will have paper copies available in the main office if parents/guardians would prefer to come into LGMS to complete. Cross Country and Girl's Basketball start the second day of school (September 6). This google form will need to be filled out prior to the first practice to be able to participate.

**ATTENDANCE INFORMATION**
Important: Student Attendance and
New Attendance Procedures
School attendance is very important, and we encourage families to make sure their children are in school as much as possible. However, children who are sick need to stay home for their well-being and to prevent others from becoming sick. Click the link to review the Lake Geneva Schools Protocols for When to Keep Your Child Home From School: https://bit.ly/3Bl6Gl9

***Students need to be to school by 7:15am and are to be in their FLEX hour by the time the bell rings at 7:25am.

Student Attendance and Attendance Procedures
One of the biggest ways parents/guardians can help their child succeed this school year is to make sure their child gets to school, on time, each and every day. The evidence is clear: children with good attendance are more likely to be successful in school. High attendance rates are linked to high student achievement. Every day counts, and missing school even 10% of the time has a significant negative impact on reading and math achievement as children progress through school. We want to ask for parent’s/guardian’s commitment to making sure their child attends school every day possible this year.

When a child does have to be absent from school, we ask that you communicate with us about the absence BEFORE 9:00AM. When your child will be absent, you have three communication options:

Call Mrs. Kramp in the main office at 262-348-3040 to speak with her and/or leave a message.

REPORT ABSENCES ON POWERSCHOOL OR A PHONE CALL OR NOW YOU CAN EMAIL THE LMGS OFFICE AT lgmsoffice@badger.k12.wi.us

BE SURE TO LET THE OFFICE KNOW BY 9:00AM.

THANK YOU!

AROUND THE DISTRICT

𝐘𝐨𝐮𝐫 𝐕𝐨𝐢𝐜𝐞 𝐌𝐚𝐭𝐭𝐞𝐫𝐬 - 𝐂𝐨𝐦𝐦𝐮𝐧𝐢𝐭𝐲-𝐖𝐢𝐝𝐞 𝐒𝐮𝐫𝐯𝐞𝐲

We are reaching out to all district residents, including parents and staff, through a community-wide survey to gather input regarding district operations and educational facility needs. The community-wide survey will be collecting responses from early November through November 22.

We have partnered with School Perceptions, an independent educational research firm, to conduct the survey and ensure all responses are kept confidential.

The survey will begin arriving through mail to households in the community around the week of November 6. Please watch for the community-wide survey in your mailbox!

Additionally, all parents/guardians and staff will receive the survey by email. Please be on the lookout for an email from School Perceptions.

We encourage every member of our community to participate in the survey and join us in shaping our schools' future.
