New dental clinic geared to patients with little means to pay


Tell your story: primary care engagement in a regional primary care alliance (LMPCA)

Please consider participating in a research study conducted by Dr Shannon Sibbald and her team at Western University.


Why?

This study aims to share the story and success of a regional primary care alliance (LMPCA) and explore primary care physician engagement. This opportunity will help shape physician engagement in London-Middlesex, and provide a chance for members to reflect, celebrate and showcase the LMPCA’s success stories to be an exemplar across provincial primary care alliances.

For your patients - free virtual self management groups in October

Please consider putting up these posters in your clinic in order to raise awareness of free virtual programs for your patients/clients/caregivers: Thames Valley FHT and Southwest Self Management Program.

Upcoming virtual programs

Sweet Talk: Patients with prediabetes and diabetes will learn how to change their health behaviours in order to improve their blood sugar control and minimize associated complications.

Heart Healthpatients with high blood cholesterol levels will learn practical approaches to managing cholesterol levels and reducing their overall cardio-vascular disease risk through health behaviour changes and the potential use of medications.

Living a Healthy Life with Chronic Conditionsfree 6 week Chronic Disease self-management program. Through the use of various coping skills, patients learn how to better care for themselves and their chronic health condition.

Getting the Most out of your Health Care Appointment: Patients learn how to prepare,  what questions to ask and how to make sure they understand everything before they leave.
