For those planning to take the LMSW Exam in New York State, please read the below information very carefully:
You will need to take the Child Maltreatment and Mandated Reporter Training:
https://www.nysmandatedreporter.org/Default.aspx
This online certification course is free and does not expire, so you only need to take it once. Please make sure to print the certificate of completion after you finish the course and keep it for your records.
Please Note: Fordham University Academic Records Department will send Form 20 on behalf of all graduating students to the New York State Office of the Professions. Form 20 replaces Form 2, so you will no longer need to complete Form 2.
If you require forms to be completed for licensing in other states or need an official transcript, please go to the National Student Clearing House. Here is the link to the National Student Clearing House https://tsorder.studentclearinghouse.org/school/ficecode/00272200 and if you have questions regarding this process, please get in touch with Academic Records at acadrecords@fordham.edu or 718-817-3900.
Best Regards,
