Dear LTHS Parents/Guardians and Families,
I hope this email finds your family having an enjoyable summer! We are less than three weeks away from the start of the 2023-24 school year and it is with great anticipation that I share some important information with our LT families.
Back-to-School Items

Complete Online Registration: The deadline for completing OLR for the 2023-24 school year was May 26. Those families that have not yet completed OLR need to do so immediately. Details are outlined within the Enrollment Services tab on the LTHS website. Targeted communications are regularly being sent to families that still need to complete OLR.

Submit Birth Certificate (Incoming Class of 2027 students and new students): Birth certificates are required for all students. The deadline for families of incoming freshman students to submit an original birth certificate to the South Campus Main Office was July 3. Those families that have not yet submitted a birth certificate need to do so immediately. The birth certificate must include the name of at least one parent. A copy will be made and the original will be returned to the family. Targeted communications are regularly being sent to families that still need to submit their child’s birth certificate.

Residency Verification: We have contracted with a company to assist the district in electronically verifying residency. Your family has been notified of your residency verification status. For those families that have not been cleared, LT South Campus will have personnel available to assist families with completing residency verification from 7:00am - 3:00pm Monday thru Friday. Specific instructions are included within residency communications. We continue to send targeted emails to families that need to verify residency. Please note that those students/families that have not had residency verified/approved prior to the start of the school year will not be able to start school on Monday, August 21. Students will be excluded from school beginning Monday, August 21 if residency has not been verified and will remain in effect until residency has been verified. Students/families who have not verified residency prior to Book Pick Up should be prepared to complete this process at Book Pick or they will not be able to get their materials/schedules.

Submit Health Paperwork:

All Incoming Freshmen Students: All incoming freshmen are required to provide: 1) Health examination with proof of immunizations prior to the first day of school. This must be completed by a MD, DO, APN or PA, dated on or after 8/21/2022 and documented on the Illinois Certificate of Child Health Examination. A separate sports physical is not needed. 2) Dental examination by no later than May 15, 2024. This dental examination must be completed by a licensed dentist on or after 11/15/22 and documented on the IDPH Dental Examination Form. We continue to send targeted emails to Class of 2027 families that still need to complete medical paperwork. If it is not on file by the first day of school, the student will be excluded from school. This exclusion shall begin immediately and shall remain in effect until the student is in compliance. Those students who have not turned in a physical prior to their book pick up appointment will be directed to meet with the South Campus Health Services Staff prior to receiving their schedule at Book Pick Up.

2023-24 Seniors, including Transition Program and 5th-year Seniors: All incoming seniors are required to provide documentation of two doses of meningococcal conjugate vaccine prior to the first day of school. You can find more information here. We continue to send targeted emails to Class of 2024 families that still need to submit documentation for the meningococcal conjugate (meningitis) vaccine. If it is not on file by the first day of school, the student will be excluded from school. This exclusion shall begin immediately and shall remain in effect until the student is in compliance. Those students who have not turned in documentation of meningococcal vaccination prior to their book pick up appointment will be directed to meet with the North Campus Health Services staff prior to receiving their schedule at Book Pick Up.

All Student-Athletes must have a current IHSA Pre-Participation Examination on file in Infinite Campus prior to tryouts. Sports physicals are valid for 395 days (13 months) from the date completed by the healthcare provider. Students and parents/guardians are able to check a student's last sports physical date in the Health Section of their Infinite Campus Portal. A separate sports physical is not required for freshmen.

Students with health needs during the school day may require additional documentation. You can find more information here.

Please submit all health documentation electronically via PDF or JPEG during online registration or to healthservices@lths.net. Please identify student name and grade level in the subject line, ie., Kathleen Weber, Grade 9. Documents will be processed in the order they are received.

Complete Fee Payment: July 3rd was the due date for online fee payment. Families that still need to complete fee payment can pay unpaid fees through the Infinite Campus Parent App. To view the individual charges and make a payment, log in to your Infinite Campus Parent App account and select the Fees option for your student(s). You may make online payments using credit or debit cards, or via e-check directly from the Parent App. If you must pay by paper check, send your payment to the North/South Bookstores Bookstore. Be sure to include your student's LT ID number on your check.

Annual Boosters and Parent Teacher Council memberships. Please consider joining and supporting these important organizations! These organizations provide outstanding support to LTHS. Additionally, they give parents/guardians an opportunity to connect with other LTHS parents/guardians.

Fee Waiver/Free Lunch Information: Information regarding application for fee waiver/free lunch can be found on the LTHS website. I am including the following information regarding eligibility for fee waiver/free lunch:
- Documentation must be submitted indicating that your household income falls within the Federal Income Guidelines (see chart on website).
- In order to apply for a fee waiver or to participate in the free lunch program, please complete one online or hard-copy application per household each school year.
- If you are applying to have your student’s school fees waived, you will also need to provide proof of income from the previous year’s Federal Tax return (1040).
- If your employment status has changed since filing, you are able to complete the form - but we may request further documentation to verify your eligibility status. Examples of supporting documentation may include a current pay stub, IDES benefits statement, notice of termination, SSI benefit statement, etc. Failure to provide documentation will delay the verification process.
- If you received direct-certification for an approved fee waiver, you do not need to fill out this application. Direct-certification letters were mailed to families indicating eligibility in July 2023.

Book Pick-Up: Lyons Township High School will welcome students on campus August 9, 10, and 14, 2023 to pick up their school issued supplies for the upcoming school year. Students are required to sign up for a time slot at the campus they will be attending. Parents are not expected to attend. Students will be picking up schedules, textbooks and Chromebooks (if they don’t already have one). Additionally, yearbook/ID pictures will be taken. Targeted emails have been sent to those students/families who have not yet signed up for book pick-up appointments. Information regarding book pick up, including the links to sign up for appointments, can be found on the Assistant Principals’ Page of the LT website.
Fall Sports Information
The fall sports season starts Monday, August 7. Those students interested in participating in fall athletics should visit the Athletics Page on the LTHS website for important information. Information regarding registration for sports and try out information is included within the Fall Sports Information Document on the website. Questions regarding tryouts should be directed to head coaches; general questions about LTHS Athletics should be directed to John Grundke, Athletic Director, at jgrundke@lths.net.
Transportation
We will be communicating important bus information to families in the near future. Families should be on the lookout for an email from South Campus Associate Principal Mr. Gardner, which will include bus information. We anticipate that bus information will be communicated no later than Friday, August 11.
Parent Teacher Council Volunteer Opportunity
Please see the following message from PTC regarding a volunteer opportunity during Book Pick Up:
Volunteers are needed on August 9th and 10th during book pick up at both campuses to supervise the PTC table and sign people up for PTC membership. Volunteers will receive a free copy of the student directory and school calendar for completing a 2 hour shift. If you already signed up for PTC Membership, you will receive an extra copy of each.
