We are delighted to present the next edition of our newsletter this December, which marks the first anniversary of the Lancet Citizens' Commission on Reimagining India’s Health System. On the International Universal Health Coverage Day (UHC Day) 2021, the Commission called for urgent actions to achieve our mission of Healthcare for All Must be Built on the Voices of All

Further, this edition explores the balance between the patient and provider behaviours and incentives in the health system. It dives into necessity of the community voices across sociocultural contexts and recognizes differential healthcare needs. 

Finally, it brings the role of, and challenges related to, utilising the AYUSH systems to attain universal health coverage in India. As a Citizens’ Commission, we invite the public to participate, provide input and continue to engage with this new initiative.

To achieve Universal Health Coverage (UHC) goals, it is imperative to include the voices of communities across sociocultural contexts, recognising the differential healthcare needs of people, writes Poonam Muttreja.
In health systems, patient and provider behaviours and incentives have to be carefully balanced. The US has tilted too much in favour of free choice. India has the opposite issue. In this piece, Nachiket Mor and Hasna Ashraf explore this issue further.

Workshops & Events
The webinar was a joint Lancet Citizens’ Commission on Reimagining India’s Health System event with Savitribai Phule Pune University (SPPU) and Dr D Y Patil Vidyapeeth (DPU). The webinar focused on the potential of and approach to Ayurveda, Yoga and Naturopathy, Unani, Siddha, Sowa Rigpa and Homeopathy (AYUSH) systems for public health in India. The panelists discussed the role of, and challenges related to utilising, the AYUSH systems to attain universal health coverage in India. 
In this webinar, the ‘Healthcare Seeking in four Indian states’ (4IS study) team discussed the emerging findings from their analysis of existing datasets, a household survey amongst 2084 households across 4 districts in UP and Odisha (conducted in Nov 2020-Jan 2021), a qualitative inquiry: in-depth interviews with 64 persons in UP and Maharashtra; and a discreet choice experiment: In UP and Odisha.
Commission Members in Spotlight 
"Our approach for UHC should be inclusive based on the public good and not only business. The reimagining attempt can be best made after considering the complexity of our systems. The Indian healthcare system should focus more on our social milieu and national priorities and leverage our strengths and resources. I feel a culturally aligned inclusive approach may be better to achieve UHC in India," says Dr. Bhushan Patwardhan, National Research Professor – AYUSH and Distinguished Professor, Savitribai Phule Pune University
"The challenges for achieving UHC in India seem to be deep-rooted such as making health a priority on the national agenda, bringing accountability at various levels in the health sector - education, research, policy and practice - and, ensuring the structures and opportunities for people to follow healthy lifestyles, be ‘active’ users and not merely passive recipients," says Sarika Chaturvedi, Scientist, Dr D Y Patil University, Pune
