Practice Spotlight: ACT Health Solutions
According to Dr. Ada Tapper, DNP, APRN, founder of ACT Health Solutions in Hillsborough County, Florida, one of her first goals is to ensure people know home-based primary care (HBPC) exists. “I want people to know we are out there to provide them with good, optimized care and to help them live their best possible life,” Tapper shared in this month’s Practice Spotlight.
 
Tapper, one of HCCI’s Home-Centered Care Champions, says she gets more out of house calls as opposed to being in an office, “I get to spend more time with patients in their environments which enables me to provide comprehensive, holistic, individualized care. In an office setting, you only see one aspect of the patient and, many times, they only tell you what they want you to know.”
American Academy for Home Care Medicine Releases Home Care Medicine Practice Directory

According to AAHCM’s press release, “the online directory helps patients and their families connect to trusted, local home care medicine providers, while also supporting the full complement of healthcare organizations who can use the directory as a resource for referrals and new care team relationships. All 50 states, the District of Columbia, and several international locations are represented.” The directory was made possible through a grant from The John A. Hartford Foundation, as part of its Age-Friendly Health Systems Initiative.

Behind the Scenes of a House Call Video Shoot
HCCI recently did a shoot for a comprehensive house call simulation video for use in our upcoming education, marketing, and other materials. It will cover all aspects of a house call, including patient assessment and examination, caregiver conversations, and a walk-through of the home to evaluate patient safety, nutrition, and other concerns. As always, the goal is to increase awareness of this type of care while educating healthcare providers on how to provide it. Stay tuned for the release of the trailer.

AAHCM Preconference Presented by HCCI
October 28, 2021 (Followed by AAHCM Annual Meeting, October 29-30)

The HCCI preconference, Level Up! Take Your House Call Operations to New Heights, coming October 28 (followed by the American Academy of Home Care Medicine’s Virtual Annual Meeting October 29-30), is designed for providers, practice managers, and other staff interested in enhancing the efficiency, productivity, and ultimately the bottom line of their house call program.
