Expanding Home-Based Primary Care: Uncovering Referral Channels, Attracting New Patients, and Partnering with Providers

Ready to grow your practice but unsure where to start? Next month’s HCCIntelligence™ Community webinar is a great first step. Available exclusively to HCCIntelligence™ Community members, this dynamic one-hour discussion will present ideas and proven strategies for practices of all sizes. The webinar is scheduled for November 20 from 3 to 4 pm CDT.

Join Home Centered Care Institute (HCCI) industry experts Paul Chiang, MD and Tammy Browning, PA-C, as they explore the multi-faceted process of identifying patient referral sources, creative ways to expand your local market presence, and how to demonstrate the value of partnering with your practice. They’ll also focus on strategies for communicating with prospective new patients and provide examples of how to present the benefits of home-based primary care (HBPC) to patients, families, and caregivers.

“There are so many important takeaways participants will gain from attending this webinar,” says Jeff Fraler, Director of Education, HCCI. “But what makes it a ‘must-attend’ event is really our presenters. Home-based primary care isn’t a sideline for them — it’s a calling, a true passion. Their knowledge, hands-on expertise, and ability to transfer what they’ve learned to others is truly invaluable for practices that want to excel and grow in the field of HBPC.”

Exclusive webinars are one of many HCCIntelligence™ Community membership benefits. Community members also enjoy access to the entire library of HCCIntelligence™ Premier Tools & Tip Sheets, free select online courses and discounted pricing on additional activities in the HCCI Learning Hub™, and personalized office hours with HCCI experts, including Certified Professional Coders (CPC)®. The Importance of Community and Storytelling in the HBPC Field

Brandon Finlen, Marketing @HCCI, shares his on-the-job insights.

When I began working with HCCI this summer, my views on home-based primary care (HBPC) were nostalgic, reminiscent of a simpler time when doctors knew all their patients by first name, and visits took place in the living room with Leave It to Beaver playing in the background. Fast forward four months, and the benefits, impacts, and challenges facing the HBPC field are abundantly clear.

Home-based primary care is a proven yet often overlooked approach that improves patients' lives by enhancing outcomes and quality of life. Practitioners and providers in this field work in a challenging environment, often wearing many hats as they balance patient care, regulatory compliance, team training, recruitment, and business development — all in the same afternoon. While the challenges are numerous, the rewards are even greater. HBPC is more cost-effective than hospital visits and generally provides better outcomes, benefiting patient care while offering our strained healthcare system a much-needed solution for an aging population.

Growing a practice may not have been a focus in medical school, yet it is a critical factor in determining whether your practice thrives or struggles. Without support, knowledge, or experience, finding success can be challenging. As the saying goes, "If you want to go fast, go alone; if you want to go far, go together." A practitioner recently shared that organizations within the HBPC field are often more supportive in helping each other succeed compared to those in the general medical industry. To provide this support and foster community, HCCI developed the HCCIntelligence™ Community, a platform filled with resources and training dedicated to solving the unique challenges that HBPC practitioners and providers face. The Community ensures that you never have to navigate these challenges alone.

Last week, I attended my first AAHCM conference, where I led a session on storytelling and its potential impact on growing practices. Stories hold a special role in the HBPC field; the richness and depth of the stories shared during the session highlight the care and dedication you bring to improving patients’ quality of life in difficult circumstances. Connecting these stories to your business can have numerous impactful benefits:

  • Redefine ROI for payers and executive leadership
  • Humanize healthcare by ensuring patients remain central to your mission
  • Promote behavior change in patients by helping them understand the benefits and outcomes through similar stories
  • Grow your practice by making your brand and quality of care more widely known
  • Improve employee retention by fostering engagement and sharing experiences, tips, and tricks across the practice

Storytelling is more than just a marketing tool; it’s a method for deepening the emotional connection between your practice and the people you serve. By understanding the power of storytelling and integrating it into every aspect of your practice — from patient care to marketing — you can drive growth and create a lasting impact in your community.

Attendees of the AAHCM storytelling session received a copy of the marketing toolkit that HCCI created to help practitioners and practices leverage storytelling to enhance both growth and patient care. If you would like to receive a copy, please email MKTG@HCCInstitute.org.
