It Takes a Village - CCR and Collaboration Across County Departments and Providers

ABOUT:
From the inception, it was clear the implementation of CCR would take a village. The reform has challenged counties to examine child welfare from a multi-disciplinary, systems approach. With this, it was clear the collaboration amongst county departments and providers would be vital in the success of CCR, and in truly improving the outcomes of children and youth. Los Angeles County, quick to realize this, made efforts to embrace collaboration from day one.

The discussion will outline the journey of CCR in Los Angeles with special attention on the collaboration between the Department of Children and Family Services, Department of Mental Health, Department of Probation , as well as with the local contracted providers . It will begin with the early days of initial CCR workgroups through current implementation strategies, highlighting both the successes and lessons learned.

ABOUT:
With the implementation of the Continuum of Care Reform (CCR) in 2017, child welfare agencies, juvenile probation, and mental health plans have adopted the Integrated Core Practice Model (ICPM) to ensure collaborative services are provided to children and families involved within the various systems. The Child and Family Team (CFT), a key component of the ICPM, serves as the primary treatment planning and decision making body. A panel discussion will focus on real-life implementation of the ICPM in Los Angeles County, taking an in-depth look at the role of the ICPM and CFT at each step in the placement and treatment process. Conversation will draw from the experiences of a previous foster youth and their parent as well as from involved community partners from the Department of Children and Family Services, Department of Mental Health, Department of Probation , as well as Hathaway Sycamores , a local contracted provider.
