Need Distance Learning for the 2021-22 School Year?

We understand that some our students may not return to 100-percent in-person learning at the start of the 2021-22 school year due to documented medical conditions. 

If your student cannot attend in-person learning due to a physical or other medical concern, please complete the COVID-19 Medical Consent and Certification for Distance Learning for 2021-22 School Year Form. This form and a list of frequently asked questions can be found HERE
 
The entire form must be completed and submitted to your FPCS campus by Friday, July 16, 2021 in order for the request to be considered for the 2021-22 school year.
 
As always, the health and safety of our learning community is a top priority.
