Dear RHRP Providers,

This memo is to inform you of recent updates made to the Vision Screening Form. Please review the following changes carefully, as they impact documentation and submission procedures:

Group 1 Vision Classification (Normal Vision):

• When a Service Member's vision acuity is classified as Group 1, questions 9-15 will no longer be required, and fields will be disabled. Providers will be able to sign and submit without needing to answer additional questions.

• If Group 1 vision classification is incorrect, providers must update BOTH EYES drop-down selections before submission to ensure the correct vision classification is assigned.

Quality Alerts:

• If the vision screening data contains any quality issues or inconsistencies, a pop-up ALERT will appear to notify the provider of the potential error. Please review and correct it before submission.

Near Vision Acuity Clarification:

• Near vision acuity is ONLY required for Service Members aged 45 years or older. Please ensure this section is completed only for the applicable individuals.

Vision Readiness Classification (Group) is based on the Visual Acuity values that are entered for BOTH EYES.

• Group 1: 20/20 or Better
• Group 2: 20/25 - 20/40
• Group 3: Worse than 20/40
