South West LHIN MRI Forms

A reminder that the SW LHIN has moved to a  SINGLE MRI REQUISITION  for all MRI's in our LHIN.

The requisitions can be found under the resources tab at the link below.

For MRIs of spine or knee, the Choosing Wisely checklist (also available on website) must also be completed and sent as of Jan 30, 2018. For all other MRIs this is not needed.

Using older requisitions will likely lead to test being declined! TIP***remove old forms from your EMRs!

Academic Detailing, Centre for Effective Practice.
Effective March 2018, family physicians in our Local Health Integration Network (LHIN) and across Ontario will be offered participation in a voluntary, free academic detailing service. This service will provide one-on-one visits with an academic detailer who will offer discussions on balanced, evidence-informed best practices, tailored to areas of physicians’ expressed interest . These visits will be provided at a time and place that is convenient for each interested physician. Each visit has been certified by the College of Family Physicians of Canada and the Ontario Chapter for up to 1 Mainpro+ credit. This service is being provided by Centre for Effective Practice (CEP), a federally-incorporated, not-for-profit organization that aims to close the gap between evidence and practice for health care providers.
To this end, the CEP would like to invite you to participate in an academic detailing visit with the detailer for our LHIN, Nicole Seymour. Nicole is a licensed Ontario pharmacist, who has received specific training in the relevant clinical evidence. The first visit that Nicole will be offering is on the topic of supporting family physicians care for their patients living with chronic non-cancer pain currently on opioid therapy.
If you are interested in scheduling an academic detailing visit with Nicole, please visit to sign-up and the CEP will be in touch with you shortly. We would encourage you to share this information with your colleagues.
