Multidrug-resistant Organisms in Long-term Care Facilities
As multidrug-resistant organisms (MDRO) become more prevalent in healthcare facilities, we are often asked how to manage individuals residing in long-term facilities who have been identified with these organisms. Utilizing case studies, we will examine potential approaches to care. The goal of MDRO management is to: 1) identify the organism, and 2) contain the organism to prevent horizontal transmission. It is important to evaluate the risk that the individual with an MDRO provides to others within the facility. Here are some key terms to understand when evaluating the risk: infection – [the process of bacteria or viruses invading the body and causing illness], colonization – [microorganisms living on or in a host without causing disease]. In either case, there is risk of horizontal transmission to others and the potential for contamination of the surrounding environment.
Case Study 1.
Mr. Richards is being transferred back from the hospital with a wound infection that was identified as multidrug-resistant
Acinetobacter baumanii. He is on a 10 day course of antibiotics and is receiving dressing changes two times a day. When the infection preventionist (IP) examines the laboratory results for this culture, she/he finds that the isolate is carbapenem resistant as well.

Transitioning to the New MDS Assessments Training
Transitioning to the New Minimum Data Set (MDS) Assessments Training has
been scheduled for September 25-26, 2019, at the U of L ShelbyHurst Campus in Louisville, Kentucky, and is open to all Kentucky providers.
This
FREE
course will review the new MDS (version 1.17.0) and Resident Assessment Instrument manual (RAI-version 1.17) updates and revisions from the Centers for Medicare and Medicaid Services (CMS) effective October 1, 2019. A significant change in the reimbursement methodology for Medicare residents will also be implemented October 1, 2019. The new methodology, Patient Driven Payment Model (PDPM), will be reviewed in this course. An overview will be provided on the Skilled Nursing Facility (SNF) Quality Reporting Program (QRP) Reports and enhancements, new and revised MDS transmission errors, and how to locate and use the MDS 3.0 Provider User’s Guide.

The Kentucky Board of Nursing has approved 12.00 contact hours for RNs and LPNs. Kentucky LTC Administrators will also receive contact hours. Participants MUST stay for the entire course in order to receive CEUs.

Pioneering a New Culture of Aging Conference
Through the generosity of a Civil Monetary Penalty (CMP) grant from the Kentucky Office of Inspector General Division of Health Care and the Centers for Medicare and Medicaid Services, you are invited to attend the free Pioneering a New Culture of Aging Conference held from August 5-7, 2019 at the Galt House in Louisville, Kentucky.
Registration and Continuing Education (if needed) costs will be covered for two team members from each nursing home in Kentucky.
One member should be either the Administrator or Director of Nursing and the other a Certified Nursing Assistant (CNA) or another staff position. The grant also covers the registration costs of 75 nursing home residents and 75 family members.
Different staff can attend on different days of the conference. One staff member does not have to attend every day of the conference.

As part of the LTC2Prepare Initiative, a password-protected website only for KY CNF has been launched
www.ltc2prepare.org. This website includes the electronic Hazard Vulnerability Analysis (HVA), information on CMS Final Rule on Emerge
ncy Preparedness, the
KY LTC Emergency Preparedness Manual in word format that is searchable, other tools to use in the facility Emergency Preparedness Program and a registration link for the Emergency Preparedness Conferences. In April, each KY CNF Administrator received a username and password that is unique to their facility. This username and password is to be shared among the staff at the facility.

Course Description:
The Methods of Instruction course prepares the participant to teach the 75 hour Medicaid Nurse Aide Course. The focus of the course is directed toward teaching the adult learner in the classroom, nursing laboratory, and clinical settings.
Eligibility for enrollment:
BSN prepared nurses with long term experience. This course is not valid for KCTCS employees or high school/secondary instructors. KCTCS employees and high school/secondary instructors require additional training that will not be offered in this course.

Reducing Antimicrobial Resistance: Shifting the Paradigm Through Antibiotic Stewardship Emphasizing Evidence-Based Practices & Interprofessional Collaboration

Provided by: University of Kentucky College of Nursing, Kentucky Department of Public Healthcare-associated/Antibiotic Resistance (HAI/AR) Prevention Program, and Atom Alliance
Objectives: The attendees of this program will be able to:
- Describe information on best practices and recommended guidelines surrounding antibiotic prescribing;
- Provide information and guidelines for implementation of Antimicrobial Stewardship programs within particular healthcare settings;
- Plan successful methods for interprofessional collaboration of prescribing providers, organizational leadership, pharmacists, and non-prescribing nursing staff.
Target Audience: All healthcare practitioners responsible for prescribing antibiotics as well as non-prescribing registered nurses, pharmacists, infection preventionists, and quality staff.

Effective October 1, 2019, CMS will be using a new case-mix model for Medicare, the Patient-Driven Payment Model, which focuses on the patient’s condition and resulting care needs rather than on the amount of care provided in order to determine Medicare payment.

Kentucky requires one member of the IDR panel to be "a person currently engaged in the provision of Long Term Care services who has no affiliation with the provider disputing a deficiency." Typically, a Long Term Care Administrator is on each IDR panel. There is no payment or travel reimbursement for an Administrator serving on a panel. Administrators will be rotated. If a Long Term Care Administrator is interested in serving on a panel, a confidentiality statement must be signed prior to the participation in the first IDR panel.
