The Department of Industrial Accidents Form 116B – Amendment to Lump Sum Agreement Pursuant to M.G.L. c. 398, §75 of the Acts of 1991 - Vocational Rehabilitation Status, has been revised in order to provide more clarity and accurately reflect the injured worker’s status.


The former form had four boxes to be completed describing the Vocational Rehabilitation status of the injured worker. However, the boxes did not apply when the injured worker was either found suitable but unable to continue with Vocational Rehabilitative Services due to a worsening of their medical condition, or the injured worker was found suitable but uncooperative which resulted in case closure.

To accommodate these circumstances and others not relevant to the existing box choices, the DIA has added a fifth box titled “Other” with space for further explanation.

On October 18, 2023, the DOL will change the email address for Memorandum of Payment (MOP) submissions to MOPSUBMISSION@DOL.NH.GOV

The old MOP email will stop working entirely and any messages sent to the old email will not be forwarded.


Also, Danielle Albert has been named Deputy Labor Commissioner and is no longer Director of the Workers’ Compensation Department. An interim director will be named soon and we will update as soon as we hear.
