We hope you are having a GREAT SUMMER! For your SUMMER HOMEWORK, please read the important information below, then complete the necessary paperwork and return it to MAP by MONDAY, AUGUST 3, 2026. Thank you!

Jump Start is a fun & enriching program for children ages 4 & 5 and not yet in Kindergarten located at our Memorial School location. HEALTH PLANS:

MAP's Health Care Policy & required forms are available on our web site: www.medfieldafterschoolprogram.com. Families of children with any chronic medical or health condition which has been diagnosed by a doctor or licensed health care practitioner, including but not limited to serious allergies, anaphylaxis, asthma, ADD/ADHD, celiac disease, diabetes, epilepsy, physical disabilities, etc. which may or may not require medical treatment and/or medication at home, school or MAP are required to complete a MAP health forms and to contact their child's program director (below). Once forms are completed, families must set up a time to meet to review the forms, discuss your child’s condition, provide training, and drop off medications (if any) between August 24-28, 2026. If these dates are not convenient, please reach out to your child's program director to arrange an earlier drop off. For the children's safety, they may not attend MAP without their completed forms & medication (if any). Just for JUMP START: The First Day of Jump Start MAP is Tuesday, September 8, 2026. Please CLICK HERE to complete a DEVELOPMENTAL HISTORY.

Please provide MAP with a copy of your child's CURRENT PHYSICAL.

MAP must have your child's completed Child Information Form, Developmental History, Current Physical, and any necessary Health Forms by

 MONDAY, AUGUST 3, 2026

Children may not attend MAP without the required forms.

NEED TO MAKE A CHANGE?

To change your child's day(s) of attendance or withdraw your child from MAP prior to the beginning of the 2026-2027 school year, please notify MAP via email to annette.map@comcast.net.

• All schedule changes must be received by Friday, 8/14/2026
• If withdrawing from the program, 50% of the tuition deposit will be refunded if notice is given by August 13, 2026. After this date, the deposit becomes non-refundable, and families will be responsible for paying the first tuition installment in full
• No schedule changes will be processed between August 14 - September 14
• Beginning September 1, families may request changes to be effective after September 14, 2026.
• After September 15, two weeks’ written notice is required to make a schedule change (add or drop days) or withdraw from the program.

TUITION STATEMENTS:

Families will receive their first Tuition Statement from MAP via email from quickbooks@notification.intuit.com on August 14, 2026 (please save to your address book and check your spam or promotions folder). Payment is due to MAP on or before September 1, 2026. Families previously enrolled in ACH, will continue to have their payment withdrawn from their account within the first 5 business days of the month. Please notify the Executive Director as soon as possible if you have a Child Care Voucher. Families without a voucher seeking tuition assistance, should contact the Executive Director at annette.map@comcast.net for more information and an application.

Per our tuition policy (available at www.medfieldafterschoolprogram.com): Tuition is due in advance. Tuition payments are payable to the "Medfield Afterschool Program" or “MAP.” Payments must be by check, via your bank’s online bill pay, money order or ACH (see below). Please include your child's name on your check. Tuition payments can be mailed to: MAP, P.O. Box 18, Medfield, MA 02052, placed in MAP’s locked drop box at the 2-3 MAP building/Main Office (located behind the Wheelock School, door closest to the playground), or dropped off at your child’s program (once in session).

ACH Payments - MAP offers families the opportunity to have their monthly tuition and any related MAP expenses debited directly from their bank account via an electronic bank to bank payment called ACH (Automated Clearing House). In order to participate in this optional form of payment, families must complete and return the Authorization Agreement and provide MAP with a cancelled check. MAP families will continue to receive a monthly tuition statement on the 15^th of each month, and tuition payments will be due on the 1^st of the following month. ACH payments will be drawn from participating families accounts in batches, starting on the 1^st and may occur within the first 5 business days of the month.

Enrollment forms and your voided check should be returned to MAP in a sealed envelope. MAP will confirm your enrollment via email.
