CHANGES TO PAYMENT BY CHECKS

Make checks payable to: DEPARTMENT OF HEALTH (Not DCAB)

At the bottom left of the check in the note/memo section include the name: Disability and Communication Access Board or DCAB
- Print a copy of DCAB's document review invoice request sent via email
- Mail check and copy of the invoice to:
Disability and Communication Access Board

Honolulu, HI 96813
Once the payment is received in-house, we will send you an email confirmation.
