According to the Georgia Department of Public Health, Georgia law requires children attending a school or a childcare facility be protected from certain vaccine-preventable diseases. Parents will need to provide Georgia Immunization Certificate (Form 3231) to their child’s school or child care to show their child has been vaccinated against certain preventable diseases. All students born on or after January 1, 2002 and entering, advancing or transferring into 7th grade need proof of an adolescent pertussis booster vaccination called Tdap and an adolescent meningococcal vaccination called MCV4.
