MEMA Storm Residential and Business Damage Form


Attention Chamber Members,


The Greater Cape Ann Chamber of Commerce is reaching out on behalf of the Massachusetts Emergency Management Agency. Due to the effects of last weekend’s storm, there is concern for coastal businesses and residents who may have been affected.


MEMA wants businesses and individuals to be aware that you can fill out their Storm Residential and Business Damage Form to document the damage to your home and/or business. This data will help determine if our communities are eligible for assistance.


Below is language from MEMA regarding this process: 


Individuals and Businesses  


Damages related to private homes and businesses will be captured using a separate survey. MEMA is utilizing an on-line IDA survey that can be filled out directly by residents and business owners. This information will be used to determine whether the Commonwealth may be eligible to request federal disaster assistance under the Individual Assistance or Small Business Administration programs and whether there is a basis for engaging in the second step of the eligibility process – formal FEMA/State Preliminary Damage Assessments (PDAs).

For assistance with the form, to submit by phone, or for access in multiple languages, residents and businesses should call Mass 2-1-1 (x20). The link will be available for form submission through Tuesday, January 23, 2024.
