Record Correction Requests



The MIIS allows providers to view patient records and make edits. Occasionally, a patient record may have an immunization that does not belong on the record. If it was reported by a provider site that differs from the site you are logged in under, you cannot delete the shot and will instead need to submit a Record Correction Request.

You may report more than one immunization at a time, but please be sure to provide details on each within the notes section. Our team may be unable to process requests without adequate information.

Managing Duplicate Patient Records



Patient records that are submitted via different sources are automatically consolidated by the MIIS to provide one source for each patient’s immunization history. There are some instances where the MIIS cannot determine if records should or should not be merged. These records are placed in the Patient Deduplication Queue for manual review by the reporting provider.

The Review and Resolve Duplicates screen will present both records that the MIIS has identified as possible duplicates. If the records belong to the same patient, you can opt to merge them, absorbing one record’s data into the record you select to maintain. 

Actively managing potential duplicates will ensure the most accurate representation of your patient population when running reports and will provide the most comprehensive data for clinical decision support.

2024-2025 MDPH Immunization Division School Surveys


The MDPH Immunization Division School Surveys are open now through January 24th, 2025. Please note the Flu Survey will not be conducted this year.


For Kindergarten, Grade 7, and Grade 12, the School Immunization Survey is available only through the Massachusetts Immunization Information System (MIIS). To access the surveys, log in to the MIIS, hover your mouse over “SCHOOL,” and select Immunization Surveys from the menu.


If you are completing the School Immunization Survey for a childcare/preschool or college, you should have received an email from the Assessment Unit containing an access link to the survey. For K-12 schools with an associated childcare/preschool, the survey link can also be accessed through the MIIS.
