2025 Patriots Difference Maker of the Year


We’re thrilled to announce that Madeline Cooke has been named the 2025 Patriots Difference Maker of the Year for her incredible dedication to Community Nurse, Inc. For the past six years, Madeline has been an unwavering volunteer, providing comfort and support to patients and their families during some of their most difficult moments. She has become a vital part of our Hospice program, offering her presence, a listening ear, and compassionate care when it is needed most.


This recognition includes a $25,000 donation from the Kraft family, the New England Patriots Foundation, and Gillette to support our mission. We are deeply grateful for this honor and for the continued support from the community and these incredible organizations. Thank you to everyone who has been part of this journey with us.

Our Annual Christmas Cactus Tradition


A heartfelt thank you to First Citizens' Federal Credit Union for their generous sponsorship of Christmas Cactus plants for our hospice patients. Because of their kindness, patients received a beautiful plant to brighten their homes and bring comfort and holiday cheer. As Melanie Howell, Director of Marketing and Community Connection at First Citizens’ Federal Credit Union, shared: “It is our pleasure to support Community Nurse with their kind and generous Christmas Cactus initiative which serves hospice patients and their families in our community. Through compassion, care, and collaboration, we all can make an impact in someone’s life. Thank you for allowing First Citizens’ to be part of the good that you do.”
