Updated Suggested SADC Site Evaluation Tool and User Guide 

The Department of Health (DOH) advises Managed Long Term Care plans that the Suggested Social Adult Day Care (SADC) Site Evaluation Tool and corresponding User Guide have been updated and added to the NYS DOH SADC HCBS Compliance website.

The updated tool and user guide have been developed as a resource to assist MLTC plans in oversight of SADC compliance with State and Federal requirements.

Changes to Tool:

For a complete list of changes made to the tool, please see the Summary of Changes to the Suggested SADC Site Evaluation Tool.

DOH states that plans should begin utilizing the updated tool and guide or ensure that your evaluation tool includes all assessment items in the DOH tool within 30 days.
