DPP Navigator Updates

After 5 years, the DPP Navigator service will be coming to an end on March 31, 2024.


What does that mean for you?


DPP Sites:

  • The DPP Navigator will no longer be sending referrals to DPP sites.
  • We will be closing all bi-directional feedback loops. This means that we will be reaching out for participant updates every 6 months until the last participant that we referred to a DPP has completed their DPP journey.

Physicians:

  • We will be reaching out to all physicians who have utilized the DPP navigator to connect them to local and state-wide DPPs where they can directly send referrals going forward.


What stays the same?


DiabetesFreeNC and the NC DPP Lifestyle Coach Network will continue to serve the DPP community with resources and networking opportunities.


The DiabetesFreeNC.com website will keep it's interactive map showing all active DPPs in the state. If you are a DPP site, check the accuracy of your site information listed on the map. Email Meg Sargent any corrections to be made to your site. DiabetesFreeNC will also continue to house valuable DPP information for sites, lifestyle coaches, and healthcare providers.


The DPP Lifestyle Coach Network will continue to connect DPPs across the state. Quarterly webinars will start up again in February. Technical assistance for DPP sites can also be requested by emailing Meg Sargent.

Free Advanced Lifestyle Coach Training Webinar

The success of the Lifestyle Change Program is the benefit to all participants in reducing or delaying their risk of developing type 2 diabetes. Ensuring that all participants meet program goals is critical to helping them reduce their risk and achieving those goals demand that we attend to our participants’ unique needs and realities. In this webinar, we will explore the many ways that programs and coaches are uncovering participant needs and addressing them through program partnerships, facilitation approaches, and resource sharing.

DPRP Office Hours - Leveraging the DSMES Fast-tracking Opportunity to Accelerate CDC-recognition for Your National DPP Lifestyle Change Program


This month’s topic will be: Leveraging the DSMES Fast-tracking Opportunity to Accelerate CDC-recognition for Your National DPP Lifestyle Change Program. This new opportunity supports ADA-recognized and ADCES-accredited DSMES delivery organizations as they launch a National DPP lifestyle change program with the hope that they will go on to become MDPP suppliers and offer the program to Medicare beneficiaries. Not only will this opportunity help organizations achieve preliminary recognition on an accelerated timeline, it also provides access to a limited supply of scholarships to cover lifestyle coach training.
