ENHANCED CARE MANAGEMENT FOR CHILDREN ENROLLED IN
ALAMEDA ALLIANCE FOR HEALTH
Children and youth in your practice may be eligible for a new Medi-Cal benefit called Enhanced Care Management, or ECM. ECM aims to help children and their families better coordinate the care they receive from multiple sources—health, mental health, social, and community-based services.
ECM does not replace any care coordination or case management a child already receives through an existing program like CCS or the Regional Center; it is additional intensive care coordination for those children and youth at highest need.
Services are provided through community-based workers with expertise in children’s services. Children who are eligible for ECM are assigned a lead care coordinator who can help connect them and their families to additional resources like food, stable housing, and other support, and help them coordinate visits to doctors and other services.
ECM is intended for children and youth up to age 21 in any of the following categories:
- Experiencing or at risk of homelessness
- Enrolled in the CCS program with additional needs beyond the CCS condition
- Experiencing serious mental health concerns and/or substance abuse
- Involved in the child welfare system such as children in foster care (former foster youth may be eligible up to age 26)
- At risk for avoidable hospitalizations or emergency room visits
Children and youth who you think need extra support through ECM should be referred to the Alameda Alliance for Health. The Alliance will determine whether a child is eligible and will contact the family to set up an intake. The Alliance encourages pediatric providers to refer any child they think may need ECM.
