HSC, Partners Help Client
End Her Homelessness
|
|
In September of 2020, "Christina’s" landlord did not renew her lease, and she became homeless. While living outdoors, her permanent residency card was stolen. Christina stayed at one of the Safe Outdoor Spaces near the Campus, and was then moved into a shelter bed at Central Arizona Shelter Services (CASS).
In May of 2021, Christina moved into Project 8, a bridge housing program operated by HSC. At the age of 73 she was experiencing a number of health issues.
HSC staff assisted Christina with setting up long-term care and an in-home caregiver - a process that can take months. While this was in the works, HSC Staff visited her often, helping her clean her room, telling jokes and stories, and assisting her with transportation to Circle the City for medical care.
HSC coordinated care with Arizona Long Term Care System for her in-home health, Social Security for her income, Homeless ID Project for her documentation, MercyCare for a shower chair, Circle the City for healthcare, and Arizona Housing, Inc. for housing. Christina secured her apartment on February 4th! HSC Staff assisted with moving her from Project 8 to her new home and provided a move-in kit, including a bed, blankets, pillow, television, pots and pans, cups, plates, coffee maker, etc. HSC also assisted Christina with move-in costs and a Fry's gift card. After she was settled in her apartment, HSC Staff took her grocery shopping. Christina was able to stock up her new kitchen and was extremely excited to cook in a full kitchen again.
Thanks to partnerships and innovative programs like bridge housing, Christina had a safe temporary place to live until she could move into her permanent home!
