Who are you choosing to carry out your wishes? Choosing a person who can act as your decision maker at a time when you cannot make decisions for yourself can be stressful, but it doesn't have to be! Start The Conversation with family and friends today!

Living Will

A Living Will does not necessarily have to be honored by EMS as the statute provides immunity to health care providers who withhold care pursuant to the wishes of the patient as expressed in this document. Be sure to discuss your options with your Health Care Representative and healthcare team.

POST

POST stands for Physician Orders for Scope of Treatment. This form is the only legally-binding form for your wishes AND has to be physically with you in your time of need. It is suggested to keep it with your medications or near your fridge for easy accessibility for emergency responders and loved ones.

She met with members of Congress and the Senate on Capitol Hill to advocate for our aging populations! TRENDING: Home Modification for Aging-in-Place

Our Aging & Disability Resource Center receives 1,500 calls every month! Recently, many calls have been regarding Home Modification services.

The most requested Home Modification services to support aging-in-place have been: ramps, grab bars and wider doors. Our trained Options Counselors are ready to assist you in exploring your options to find the most appropriate home modification for you or your loved one.

When calling in, you may be asked some questions to understand the condition-specific need for Home Modification services, such as:

What has been happening?
How long has this been going on?
Have you talked to your doctor about this occurrence?
