Yearbook reporting is continues. The deadline to submit reports has been extended to April 11.


Thank you to the churches who have already submitted their data; I've heard from national that around 70 of our churches have already reported. That is 1/3 of our Wisconsin Conference churches!


For those churches who have yet to report, I would like to address a few common questions I've been getting (FAQs section below), and am also including the original instructions (see original message at the bottom of this email/section with the "Yearbook It!" image). But the most important thing to note is this year's Yearbook collection system is different than in past years; your email and password from last year will not work. Please see the directions for info on how to sign in to the new system as a Yearbook Reporter.

Yearbook FAQs


  1. Nola's correct phone number is 224-258-0364.
  2. The access code is "graceabounds". To create an account for your church to log in, you need this code plus your church ID number.
  3. Printing of Yearbook reports currently is not working, but the national office is working on it.
  4. Churches may not seeing any staff as assigned to their church. When I go to check on this, the information is always there on my end. Do not worry if you don't see your pastor listed; it is just not showing for yearbook reporters at this time.
  5. All information for "UCC special offerings" will be reported to national by the Conference at a later date.
  6. When you've completed reporting, it will say 5/6 sections are completed. That is correct. The 6th section is the special offerings that are reported by the Conference.
  7. If you enter info and then stop before you've finished, you can go back in later. But, you must sign in and go to your church, then scroll down the page to the yearbook section and click on the year 2024. From there, click on the ellipses button in the top right-hand corner and select "continue reporting."
  8. If you have trouble with this new system, the UCC has developed a new way to report this year: a Survey Monkey option that will collect all of the data that is normally collected from churches, available here: https://www.surveymonkey.com/r/UCC_Backup_YB_Data_Entry_2025

It’s time for the year-end Yearbook forms. The national office of the UCC has revamped their Data Hub website. This was just recently completed following some delays, so accessing the site is a bit different this year. Also, the reporting dates have been delayed. The start date is today, and the submission deadline is April 11.


A paper mailing from the Conference was mailed to your congregation with instructions for submitting yearbook data. This email serves as a reminder of this mailing and includes clickable links to the information from the mailing. IMPORTANT NOTE: The church yearbook instructions were updated by national AFTER this mailing was sent out. Please refer to the instructions linked here.

  • The Yearbook webpage provides resources to help churches in submitting annual church information online directly to the Data Hub. The page contains all instructions and video tutorials to assist you with submitting the Yearbook data.
  • To find your church’s six-digit ID number, you can either go to this webpage (the churches are sorted by the city of the mailing address of the church) or you can find the ID number on the left-hand corner of the mailing label that was sent to your church.
  • For system log in instructions, refer to the updated church yearbook instructions.
  • The Supplemental Yearbook Survey has a few supplemental research questions. This is a brief survey (should take no longer than 10 minutes to complete).
  • The Church Officers Report is used by the Conference and we greatly appreciate your taking the time to fill out this survey.
