The EI Family Survey (also known as the NCSEAM Family Survey) is online!
If your child has been enrolled in Early Intervention (EI) for at least six months and you did not fill out a Family Survey in the last year, you may be eligible to share your feedback.
If you are eligible, you will be given a one-time user code to log on and complete the Survey. Please fill it out and hit send when you have finished. Paper copies are also still available!
Your feedback helps the Department of Public Health (DPH) and your EI program improve services for your child and family, and for other families as well.
